Register With The Practice?

Please bring your medical card to the surgery if possible. If you cannot find it, we will give you a form to fill in, but your records will take longer to reach us without your original medical card. All newly registered patients over five years of age will be required to see the health care assistant for a simple medical examination, including blood pressure, urine testing, height and weight.
